POSITION OVERVIEW

St. Stephen's/House of Charity has been awarded a State Opioid Response (SOR) grant. The Housing Navigator will work within this grant program to assist clients with housing resources while they are actively engaged in our Substance Use Disorder Treatment; Day by Day outpatient Program for opioids. This position is responsible for working directly with these individuals, working closely to identify potential affordable, secure, stable housing. This position provides advocacy to participants while in treatment through the housing process and provides housing case management and resource navigation.

PRIMARY DUTIES AND EXPECTATIONS

  • Assess, build and maintain a caseload of 15-20 eligible individuals for the SOR program based on predetermined criteria-including vulnerability assessments, measurable progress in SUD treatment.
  • Assist clients with moving out of the shelter by working collaboratively with other departments within St. Stephen's Human Services, Hennepin County Coordinated Entry System, as well as other community partners and landlords.
  • Meet with SUD treatment patients to gauge their involvement and progress
  • Provide case management services conduct home visits; provide landlord-tenant support; support client goal planning; provide transportation as needed. Maintain active, current affordable housing resource lists
  • Assist in helping participants acquire housing related skills to avoid losing such housing
  • Recruit and network with other agencies and individuals to help access supportive services and housing including employment training, landlords, employers and volunteer opportunities for participants.
  • Maintain participant files and develop reports as required by the grant.
  • Ensure program outputs and outcomes are achieved in accordance with appropriate funding sources.
  • Assist participants access various other needed services such as but not limited to public assistance, General Assistance (GA), Minnesota Supplemental Aid (MSA), Minnesota Family Investment Program (MFIP), Social Security Benefits, Veterans Benefits, Food Support, Housing Support and any other support programs.
  • Attend all assigned and planned staff meetings, conferences and trainings.
  • Assist clients in making progress on self-identified goals using the housing first/harm reduction model.
  • Build and maintain professional rapport with clients.
  • Manage incoming inquiries for assistance and make appropriate referrals.
  • Other duties as assigned.

  • Ability to work respectfully with clients, coworkers, and key stakeholders who are diverse with respect to race, ethnicity, gender identity, sexual orientation, socio-economic status, and religion.
  • Ability to establish rapport quickly with people experiencing a crisis through use of trauma informed care, client centered care, and strengths-based approaches.
  • Ability to meet expectations regarding paperwork and documentation through organizational skills, a strong attention to detail, and the ability to work efficiently and accurately, competency and comfort using technology and computers.

Experience that gives applicants the required skills, abilities, and perspectives listed above are commonly gained through one or more of the following:

  • Personal experience with homelessness;
  • Experience working, interning, or volunteering with people experiencing homelessness;
  • An Associates, Bachelor's, or Master's degree in a relevant field such as Social Work (with accompanying licensure), Human Services, or Family Studies.

This position requires use of your own personal vehicle for Agency business and will be required to maintain liability insurance coverage of at least $100,000 per person and $300,000 per occurrence.

In addition, applicants must:

  • See themselves as a change agent, have a collaborative nature, and bring a positive attitude to work each shift.
  • Have a Bachelor's degree or 4 years of experience in a related field such as homelessness/housing, corrections, chemical or mental health care.
  • Be comfortable working in high energy settings.
  • Have the ability to communicate effectively, positively and professionally with participants, providers, family members, and others while maintaining confidentiality.
  • Must have strong personal and professional boundaries.
  • Have a demonstrated ability to de-escalate volatile situations and a customer-oriented approach in all situations.
  • Have broad knowledge of housing resources.
  • Have knowledge of and commitment to Harm Reduction/Housing First Model.
  • Have proficiency in prioritizing and multi-tasking while maintaining accuracy, organization and attention to detail.
  • Be comfortable using a computer, including Microsoft Office products.
  • Have a valid MN Driver's License and reliable transportation with the ability to transport clients when necessary.
  • Must pass background check.

PREFERRED QUALIFICATIONS

  • Spanish or Somali proficiency.

PHYSICAL REQUIREMENTS

  • Light work - Mostly standing with some sitting. Occasionally exerting up to 20 pounds of force.
