St. Stephen's Housing Programs Department serves households who are experiencing or at risk of homelessness in Hennepin County. Our programs include Prevention, Diversion, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for households with children and adults without children. By offering programs of various intensity and length, we hope to be able to provide the level of service each household needs to be successful. The goal in all of our housing programs is to end homelessness by assisting households in stabilizing their housing and achieving self-sufficiency.

The Diversion Case Manager position is a member of the Early Intervention team whose shared objective is to interrupt households' homelessness as early as possible. The Diversion Case Manager helps families who have sought shelter work toward housing stability by assisting families to find immediate alternate housing options in order to safely divert them from entering the shelter system. This position will work creatively and collaboratively with families to seek safe alternate housing options, provide short term case management, and, if necessary, provide brief and flexible financial assistance to resolve immediate challenges a family may be facing.

The Case Manager, alongside the Program Manager and other Diversion Case Manager, will monitor spending and services to ensure that program requirements are fulfilled and the programs meet the budgetary and contractual expectations.


  • Provide case management for families to assist them with stabilizing their housing (intake, assessment, goal planning, and regular contacts including home visits*).
  • Maintain regular contact with landlords, negotiate payment arrangements, and mediate landlord-tenant issues.
  • Provide financial assistance for participants in compliance with program requirements and budget.
  • Assist with program reporting, ensuring that program is on track to meet contractual goals.
  • Coordinate with Early Intervention Program Manager to ensure compliance with funding expectations.
  • Establish and maintain active working relationships with community partners and other providers.
  • Work cooperatively and collaboratively with other agency staff and other community partners to meet the needs of the client.
  • Connect clients to mainstream resources that will continue after the termination of our services.
  • Ensure clients have access to information regarding tenant responsibilities and rights, financial management, and affordable housing opportunities.
  • Complete accurate case notes, data collection, and other documentation for assigned clients in a timely manner.
  • Attend relevant community meetings.
  • Other duties as assigned.

This work consists approximately of 50% direct client contact*, and 50% indirect client-related work (i.e. documentation, communication with partners, preparing for client meetings, attending trainings and community events)

*Please note that currently due to COVID-19, safe physical distancing measures are in place for case management services.

  • Ability to work respectfully with clients, coworkers, and key stakeholders who are diverse with respect to race, ethnicity, gender identity, sexual orientation, socio-economic status, and religion.
  • Ability to establish rapport quickly with people experiencing a crisis through use of trauma informed care, client centered care, and strengths-based approaches.
  • Ability to meet expectations regarding paperwork and documentation through organizational skills, a strong attention to detail, and the ability to work efficiently and accurately, competency and comfort using technology and computers.

Experience that gives applicants the required skills, abilities, and perspectives listed above are commonly gained through one or more of the following:

  • Personal experience with homelessness;
  • Experience working, interning, or volunteering with people experiencing homelessness;
  • An Associates, Bachelor's, or Master's degree in a relevant field such as Social Work (with accompanying licensure), Human Services, or Family Studies.

This position requires use of your own personal vehicle for Agency business and will be required to maintain liability insurance coverage of at least $100,000 per person and $300,000 per occurrence.

In addition, applicants must:

  • Have a valid Driver's License and pass a DHS NetStudy 2.0 background check and a driving record background check.


  • Two years' experience working with people experiencing homelessness.
  • Fluency in languages used by our clients (Spanish, Amharic, Oromo, Somali, ASL)
  • Conflict Mediation education, knowledge, and/or experience.


  • Sedentary work Sitting most of the time with occasional walking. Exerting up to 10 pounds of force occasionally.
  • Must be able to attend home visits and apartment showings with clients, including units that may only be accessible by climbing stairs
